Child Loyalty Plan ($290/Year for ages 1-13)
Includes:
- 2 Exams
- 1 Emergency exam
- 2 Cleanings
- 2 Fluoride treatments (child only)
- 4 Bitewing X-rays
- 2 Periodontal screenings
- 15% Off additional treatments
- (A $721 value!)
Plans are active upon enrollment for 12 month period during which the plan benefits will be provided. Cannot be combined with Insurance or Care Credit. Plans will auto-renew annually. Benefits do not roll over from year to year. Lapsed plans subject to re-enrollment fee.
